2. No Emergency or Crisis Services

This website is not intended for crisis or emergency use.

3. Use of Online Forms & Scheduling Tools

Our website may allow you to:

  • Submit contact forms

  • Request appointments

  • Access secure client portals

  • Participate in telehealth services

Submitting a form or scheduling request does not establish a client–therapist relationship and does not guarantee acceptance into therapy.

Do not include highly sensitive personal or clinical information in basic website forms or email. For clinical communication, use the secure client portal only.

4. User Responsibilities

By using this website, you agree that you will:

  • Provide accurate, honest information

  • Use the site only for lawful purposes

  • Not attempt to bypass security or interfere with site functionality

  • Not upload or transmit harmful software, viruses, or malicious code

You must be at least 18 years old, unless engaging with the site under parental or guardian supervision for the purpose of seeking services for a minor.

5. Privacy & HIPAA

Your use of this site is also governed by our Privacy Policy.

For clients receiving services, the use and disclosure of Protected Health Information (PHI) is governed by our HIPAA Notice of Privacy Practices (NPP), provided separately.

Website data (such as contact form submissions or analytics) may not be PHI unless transmitted through HIPAA-compliant systems.
